Home-Based Primary Care Services

AI and Five, Inc. provides comprehensive primary care services to adult and geriatric patients in the comfort of their homes. Services include preventive care, chronic disease management, acute illness evaluation, medication management and reconciliation, annual wellness visits, post-hospital follow-up, laboratory and diagnostic test review, and coordination of care with specialists, caregivers, home health agencies, and other healthcare providers.

Home visits are the primary mode of care. Telemedicine visits are available on an occasional basis when clinically appropriate.
