AI and Five, Inc. provides personalized primary care for adults and older adults in the comfort of their homes. Our goal is to make high-quality healthcare more accessible by delivering comprehensive medical care directly to our patients.

In-Person Home Visit Service Area

We currently provide in-person home visits by appointment in:

• Brooklyn, New York

• Staten Island, New York

Home visits are scheduled based on clinical need, provider availability, and location.

Telemedicine Services

For established patients when clinically appropriate, telemedicine appointments are available throughout New York State.

Virtual visits may be appropriate for:

• Follow-up appointments

• Medication management

• Review of laboratory and diagnostic test results

• Chronic disease management

• Certain non-urgent medical concerns

Not all medical conditions are appropriate for telemedicine. If an in-person evaluation is necessary, we will discuss the best options for your care.
