What is home-based primary care?
Home-based primary care brings comprehensive medical care directly to your home. Instead of traveling to a medical office, your healthcare provider visits you in the comfort of your home to provide routine and ongoing primary care.
Who can become a patient?
AI and Five, Inc. provides primary care for adults and older adults who may benefit from receiving healthcare at home. This includes individuals with mobility limitations, chronic medical conditions, difficulty traveling, or those who simply prefer the convenience of home-based care.
Where do you provide home visits?
We currently provide in-person home visits throughout:
Brooklyn, New York
Staten Island, New York
Do you offer telemedicine?
Yes. Telemedicine appointments are available for established patients throughout New York State when clinically appropriate. Virtual visits may be used for follow-up care, medication management, review of laboratory results, and certain non-urgent medical concerns.

What should I expect during my first visit?
Your initial visit typically includes:
Review of your medical history
Discussion of your current health concerns
Medication review
Comprehensive physical assessment
Development of an individualized care plan
Time to answer your questions and discuss next steps
Do you coordinate care with my specialists?
Yes. We work closely with specialists, home health agencies, therapists, pharmacies, hospitals, caregivers, and family members to help ensure coordinated, patient-centered care.
Can family members or caregivers participate in visits?
Absolutely. With your permission, family members and caregivers are encouraged to participate in visits. Their involvement often helps improve communication, support treatment plans, and ensure continuity of care.
Do you prescribe medications?
Yes. When medically appropriate, prescriptions and medication refills are managed as part of your ongoing primary care. We also review your medications regularly to help ensure they remain safe and effective.
Do you order laboratory tests or diagnostic studies?
Yes. Laboratory tests and other diagnostic studies may be ordered when medically necessary. We review your results with you and explain any recommended treatment or follow-up.
Do you provide emergency care?
No. AI and Five, Inc. does not provide emergency medical services.
If you are experiencing chest pain, severe shortness of breath, signs of a stroke, severe bleeding, or another medical emergency, call 911 or go to the nearest emergency department immediately.
Do you accept insurance?
Please contact our office to discuss insurance participation, payment options, and any questions regarding coverage. We will be happy to review your individual situation.
