My name is Ariel Steinherz, and I founded AI and Five, Inc. with a simple goal—to provide compassionate, high-quality primary care that meets patients where they are most comfortable: in their own homes.

I believe healthcare should be personal, accessible, and built on trust. Every patient deserves to feel heard, respected, and actively involved in their healthcare decisions. By caring for patients in their homes, I am able to better understand their individual needs, build meaningful relationships, and provide medical care that is truly centered around the person rather than the office visit.

My Journey

My career in healthcare and human services spans more than 25 years, beginning with supporting individuals with developmental disabilities before becoming a registered nurse and ultimately a board-certified nurse practitioner.

Over the years, I have cared for patients in home health, primary care, rehabilitation, and long-term care settings. These experiences have strengthened my belief that excellent healthcare goes beyond treating illness—it requires listening, educating, collaborating, and developing lasting relationships with patients and their families.

Today, my practice focuses on providing comprehensive home-based primary care for adults and older adults throughout Brooklyn and Staten Island, with telemedicine services available throughout New York State when clinically appropriate.

Rather than focusing solely on treating illness, I strive to help patients prevent complications, manage chronic conditions effectively, and improve their overall quality of life.

Beyond the Office

Although my work as a nurse practitioner is a significant part of my life, I also value spending time with family and friends, continuing my professional education, and exploring ways technology can improve healthcare delivery.

I have a strong interest in healthcare innovation and enjoy learning how technology and artificial intelligence can enhance communication, improve efficiency, and support better patient care—while always preserving the personal relationships that are at the heart of medicine.
