About
Welcome to AI and Five, Inc.
I am a board-certified Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP-BC) with more than 25 years of healthcare experience and over a decade of nursing practice. My passion is providing compassionate, high-quality primary care to adults and older adults in the comfort and convenience of their homes.
I earned my Doctor of Nursing Practice (DNP) from Gannon University in 2024 and my Master of Science in Nursing from the College of Staten Island. Throughout my career, I have cared for patients across home care, primary care, rehabilitation, and long-term care settings, with extensive experience in chronic disease management, preventive care, wound care, medication management, post-hospital follow-up, and care coordination.
As a home-based primary care provider, I believe healthcare should be accessible, personalized, and centered around each patient’s individual needs. I work closely with patients, families, caregivers, specialists, and home health agencies to develop treatment plans that promote independence, improve quality of life, and help patients remain safely at home whenever possible.
Services include:
Home-based primary care
Annual Wellness Visits
Chronic disease management
Acute illness evaluation
Medication management and reconciliation
Preventive care
Wound assessment and management
Care coordination
Occasional telemedicine follow-up when clinically appropriate
My goal is to provide thoughtful, evidence-based care while building long-term relationships with patients and their families through compassionate, accessible healthcare delivered where patients are most comfortable—their home.
