Comprehensive Home-Based Primary Care

At AI and Five, Inc., we provide personalized, comprehensive primary care for adults and older adults in the comfort of their own homes. Our mission is to deliver compassionate, evidence-based medical care that promotes health, independence, and quality of life while reducing the burden of traveling to a medical office.

Home visits are our primary method of care. Telemedicine appointments are available on a limited basis when clinically appropriate for established patients.

Primary Care House Calls

Annual Wellness Visits

Medicare Annual Wellness Visits focus on prevention and maintaining your long-term health.

These visits may include:

• Review of your medical history

• Personalized wellness planning

Medication Management

Taking multiple medications can be challenging. We help ensure your medications are safe, effective, and appropriate.

Services include:

• Medication review and reconciliation

• Evaluation for side effects and drug interactions

• Prescription management

• Patient and caregiver education

Acute Illness Evaluation

When new medical concerns arise, we evaluate symptoms promptly and develop an appropriate treatment plan whenever possible in the home.

• New or worsening medical concerns

If emergency care is needed, we will guide patients to the appropriate level of care.

Wound Assessment and Management

We provide evaluation and ongoing management of acute and chronic wounds, including:

• Pressure injury evaluation

• Venous and diabetic wound monitoring

• Dressing recommendations

• Coordination with home health agencies and wound care specialists when appropriate

Post-Hospital and Transitional Care

The period after hospitalization is critical to recovery.

We provide:

• Hospital discharge follow-up

• Medication reconciliation

• Review of discharge instructions

• Monitoring for complications

• Coordination with specialists and home health providers to help reduce unnecessary hospital readmissions

Care Coordination

Quality healthcare involves collaboration. We communicate with other members of your healthcare team to ensure continuity of care.

We coordinate care with:

• Medical specialists

• Home health agencies

• Physical, occupational, and speech therapists

• Pharmacies

• Diagnostic laboratories

• Caregivers and family members

• Hospice and palliative care providers, when appropriate

Laboratory and Diagnostic Review

We review laboratory results and diagnostic testing with you, explain findings in understandable language, and adjust your treatment plan when necessary.

Telemedicine

While home visits remain our primary focus, telemedicine appointments may be available for established patients when clinically appropriate. Virtual visits can be helpful for selected follow-up appointments, medication reviews, and certain non-urgent concerns.
