Our goal is to make receiving medical care as convenient and stress-free as possible while building lasting relationships with our patients and their families.

Who We Serve

Our practice provides home-based primary care for adults and older adults who may benefit from receiving medical care at home, including those who:

Have difficulty traveling to a medical office
Have chronic medical conditions requiring ongoing management
Are recovering after hospitalization or rehabilitation
Prefer the convenience and comfort of home-based care
Require coordination with caregivers or home health services

Becoming a New Patient

Getting started is simple.

Step 1: Contact Our Office

Call 917-370-4556 or email arielnp@aiandfive.com to discuss your healthcare needs and determine whether our services are the right fit for you.

Step 2: Initial Conversation

We will answer your questions, review your medical needs, verify that you are located within our service area, and discuss scheduling your first home visit.

Step 3: Complete New Patient Forms

Before your first appointment, you may be asked to complete registration and consent forms and provide insurance information, if applicable.

Step 4: Your First Home Visit

During your initial visit, we will:

Review your medical history
Discuss your current health concerns
Review your medications
Perform a comprehensive health assessment
Develop an individualized care plan
Answer your questions and discuss next steps

What to Have Available

To help us provide the best possible care, please have the following available during your first visit:

Photo identification
Insurance card(s), if applicable
Current medication list or medication bottles
Recent hospital discharge paperwork, if available
Medical records or specialist information, if available
Contact information for your pharmacy
Emergency contact information
Advance directives or healthcare proxy documents, if available

Caregivers and Family Members

Family members and caregivers are welcome to participate in appointments with the patient’s permission. We believe that involving caregivers helps improve communication, supports treatment plans, and promotes better health outcomes.

Telemedicine

Home visits are the primary method of care. Telemedicine appointments may be available for established patients when clinically appropriate and when an in-person visit is not necessary.

Emergencies

AI and Five, Inc. does not provide emergency medical services.

If you are experiencing chest pain, difficulty breathing, signs of a stroke, severe bleeding, or another medical emergency, call 911 or go to the nearest emergency department immediately.
