Home visits are provided by appointment only.

To request a new patient appointment or learn more about our services, please call or email us. We will discuss your healthcare needs, confirm that you are within our service area, and schedule a visit if appropriate.

Office Visits

AI and Five, Inc. is a home-based primary care practice. We do not maintain a public office for patient visits. Medical care is provided in the patient’s home by appointment.

Emergencies

AI and Five, Inc. does not provide emergency medical services.

If you are experiencing a medical emergency, including chest pain, difficulty breathing, signs of a stroke, or severe bleeding, call 911 immediately or go to the nearest emergency department.
